9. Location the incident occurred - please be as specific as you can

Please be specific and detailed and don't worry about writing too much. Information that helps us on this report includes:

- Who was involved

- In what capacity were they involved

- What was happening before the incident

- What happened after the incident

- What was the behavior that didn't feel right to you
